Finding a PPO Healthcare Provider

If your medical plan includes the use of designated "Preferred Providers" for healthcare services, you must choose from the list they provide. Refer to your "Plan Identification Card" for the name of the PPO your organization has chosen then follow the link below to the appropriate company for a list of eligible physicians, hospitals and other health care providers.
